Week 19 of Pregnancy: What to Expect

Week 19 is when exercise starts to feel different. A hormone called relaxin loosens the ligaments, so the joints that used to hold you steady no longer quite do. Sleep also gets harder around here, and the fixes are all mechanical.

Your body this week

Relaxin loosens ligaments across the body in preparation for birth, and the immediate effect is that the back, knees and ankles have less support than usual. Running and other high-impact exercise often becomes uncomfortable as a result.

Staying active is still recommended. Both NHS and ACOG guidance support regular activity through pregnancy for most women, with adjustments rather than stopping — walking, swimming and low-impact classes rather than pounding pavements.

For sleep, the NHS suggests sleeping on your side, using pillows to support the bump and aching muscles, and putting a pillow between the knees. Side sleeping becomes a safety point rather than a comfort one from 28 weeks, so building the habit now is worth the effort.

What you may notice

Tiredness and sleep problems, stretch marks, sore gums, round ligament pains, headaches, nosebleeds, pelvic pain, bloating and constipation, indigestion and heartburn, sore breasts, leg cramps, feeling hot, dizziness, swollen hands and feet, urine infections, vaginal infections, and skin and hair changes.

Pelvic pain that makes walking or turning in bed difficult is worth naming at your next appointment — it is common, it has a name, and there is physiotherapy for it.

Your baby this week

Around week 19, the figure given is about 15.3cm from head to bottom — beef tomato-sized, in the NHS's comparison. It is an average across many pregnancies and not a measurement of any one.

Adult teeth are forming, lining up behind the first set that will come through in infancy. Weight is going on steadily now, which is most of what the next twenty-one weeks are about.

What care usually happens around now

The anomaly scan is offered between 18 and 20 weeks in England, so it often falls this week. In the UK the second-trimester screening blood test window closes at 20 weeks.

Nothing else is routinely scheduled. If you have not yet talked about where you would like to give birth, services generally expect that conversation somewhere in this trimester, and the options available vary between countries and areas.

This week's tip

Put a pillow between your knees tonight. It is the cheapest intervention in this entire trimester, it takes the strain off loosened hip and pelvic joints, and it is the single change most women say made side sleeping bearable rather than theoretical.

When to see a doctor

Report bleeding, severe pain, a fever, or a headache with vision changes. Pelvic pain that limits walking should be raised at your next appointment rather than endured. Pregnancy warning signs has the full list.

Can I keep running?

Many women do for a while and many stop because it stops being comfortable. Guidance supports staying active with adjustments; ask your midwife about your own situation rather than a general rule.

Why does my pelvis hurt when I turn over in bed?

Loosened ligaments are the usual reason, and it is common enough to have dedicated physiotherapy. Week 20 is the halfway point, and week 18 covers the scan.
